Jaya, 9 sem Roll no 47 I've been given this case to solve in an attempt to understand the topic of "Patient clinical data analysis" to develop my competency in reading and comprehending clinical data including history, clinical findings, investigations and come up with a diagnosis and treatment plan. Date of admission : 29/03/2022 CASE : A 65 year old female, herder by occupation, bought to the opd with chief complaints of ~Generalised weakness since 1year ~Pain of right fingertips since 2 months ~Dryness of mouth and inability to swallow since 2 months ~H/o fall on 28/3/22 at 8 PM and 29/3/22 at 3 AM HOPI : • Patient was apparently asymptomatic 4 years back.
